Kala Studio - Privacy, Booking, and Service Liability Waiver

Welcome to Kala Studio. We are committed to delivering high-quality brow and beauty services in a safe, transparent, and respectful environment. Before proceeding with any services, we kindly ask that you review and consent to the following comprehensive Privacy, Booking, and Liability Waiver Policy. These terms are designed to protect both our valued clients and our trained service providers.

 

GENERAL WAIVER & CONSENT

By booking an appointment with Kala Studio (operating under XtraMile Group LLC), you acknowledge and voluntarily consent to receive aesthetic services. You understand that our services are non-medical in nature and are intended for cosmetic and self-care purposes only. All clients must disclose relevant medical history, allergies, medications, or skin sensitivities prior to services.

 

Clients under the age of 17 must have a parent or legal guardian present to authorize any service.

 

FACIAL SERVICES WAIVER

I understand that I am electing to undergo facial and/or skin treatments with the understanding that results vary based on individual factors such as skin condition, age, and adherence to post-treatment care. I confirm that:

  • I have disclosed all relevant medical history, including allergies, medications, or topical treatments.

  • I understand there are no guaranteed outcomes.

  • I will follow all aftercare instructions and communicate any issues immediately.

  • I release Kala Studio and its estheticians from liability for undisclosed conditions or non-compliance with aftercare.

 

BODY & FACE WAXING WAIVER

I understand that waxing carries potential side effects, including redness, swelling, or skin sensitivity. I confirm that:

  • I have not used Retin-A, Accutane, glycolic/alpha hydroxy acids, or other skin-thinning agents in the last 72 hours.

  • I am not currently diabetic or immunocompromised.

  • I have disclosed any allergies or contraindications.

  • I release Kala Studio from liability related to reactions caused by undisclosed information or post-care noncompliance.

 

TINTING, HENNA, AND LAMINATION WAIVER

I acknowledge that brow/lash tinting, henna, and lamination treatments carry certain risks, including:

  • Eye irritation, allergic reactions, or skin sensitivity

  • Unintended color absorption or fading

  • Temporary staining of skin

 

I consent to these services, understanding that:

  • Reactions are possible even with "natural" or plant-based products

  • I have disclosed all allergies and medications

  • I will seek immediate medical care if adverse reactions occur

  • I hold Kala Studio harmless for unforeseen results when accurate information has been provided

 

LASH EXTENSION WAIVER

I understand that eyelash extension application and removal involve adhesives and tools near the eyes. Potential risks include:

  • Irritation, allergic reaction, or discomfort

  • Temporary or permanent lash loss

  • Eye damage in rare cases

 

I confirm that:

  • I assume full responsibility for care and maintenance

  • I will follow pre/post-care instructions

  • I release Kala Studio and its technicians from liability due to adverse outcomes, except in cases of gross negligence

 

MICROBLADING SERVICES WAIVER

Microblading is a semi-permanent procedure involving pigment implantation into the skin. I understand that:

  • Results vary depending on skin type, lifestyle, and adherence to aftercare

  • Temporary redness, swelling, or bruising may occur

  • Additional sessions may be required for optimal results

  • Pigment may remain visible for 1-3 years or longer

  • All hygiene standards will be upheld and sterile tools will be used

 

I have read and received aftercare instructions and understand that:

  • No exact color or shape is guaranteed

  • This is a cosmetic tattoo procedure and results are subject to individual variation

  • I release Kala Studio and its staff from liability for reactions caused by inaccurate disclosure or neglect of aftercare

 

CANCELLATION & REFUND POLICY

To support our team and provide fair access to all clients:

  1. Cancellations must be made at least 24 hours in advance. Late cancellations or no-shows may result in a 20% fee.

  2. Emergency cases will be reviewed individually. Please communicate with us as early as possible.

  3. Refunds are not granted for dissatisfaction unless an error in service quality is confirmed. In such cases, we may offer corrective sessions.

  4. Rescheduling is allowed up to 24 hours prior to your appointment.

  5. Open communication is encouraged. Contact us with any concerns so we can resolve them promptly.

 

MARKETING & PRIVACY DISCLOSURE

Kala Studio uses Uzeli Live (powered by Growthzilla) for customer engagement. When opting into SMS or email communications in-store or online:

  • Your personal data is used only for booking reminders, service updates, and promotional messages

  • You can opt out of SMS by replying STOP or OFF

  • You may unsubscribe from emails via the link provided

  • We do not sell or share your personal data with third parties for commercial use

ACKNOWLEDGEMENT & CONSENT

By booking a service with Kala Studio, I certify that:

  • I have read, understood, and agree to all policies listed

  • I have disclosed all medical and personal information to the best of my knowledge

  • I voluntarily release Kala Studio and XtraMile Group LLC, its owners, staff, and affiliates from liability related to treatments performed based on accurate disclosures

  • I understand this constitutes full disclosure and agreement
